What is the PPBS test?

Post-prandial blood sugar (PPBS) is blood glucose measured two hours after the start of a meal. It shows how well your insulin handles a real food load, which is a different question from the one a fasting sugar answers.

In early type 2 diabetes the after-meal sugar goes wrong first. The fasting value can be normal while the two-hour value is already high, so PPBS catches diabetes and prediabetes that a fasting test misses. Below 140 mg/dL is normal. Between 140 and 199 mg/dL suggests impaired glucose tolerance, and 200 mg/dL or more on two occasions is in the diabetic range. For someone already on treatment, the PPBS shows whether meal-time tablets or insulin match the food, and it drives most of the HbA1c once the fasting sugar is controlled.

The test itself needs no fasting, but timing matters. Eat your usual breakfast, note the time of the first bite, and the phlebotomist collects the sample two hours later at home. It is usually paired with a fasting sample from the same morning. The report arrives on WhatsApp and email in 8 hours.

How to prepare

  • No fasting is needed for this test; if a fasting sugar is booked too, give that sample first and then eat.
  • Eat a normal breakfast, not lighter or heavier than usual, and note the time you took the first bite.
  • The sample must be collected 2 hours after that first bite, so book the slot accordingly.
  • Do not eat, snack or smoke in those two hours, and stay at rest; take your usual diabetes medicine with the meal unless your doctor says otherwise.

Who should take it

Anyone whose fasting sugar was borderline (100 to 125 mg/dL) or whose HbA1c was in the prediabetes range. People with symptoms of high sugar: thirst, frequent urination, tiredness, blurred vision, slow-healing wounds. Adults over 35 with a family history of diabetes, excess weight, PCOS or high blood pressure. Everyone with diagnosed diabetes, usually every three months with a fasting sample, or more often while the doctor adjusts medication. Pregnant women being monitored for gestational diabetes.

Before the visit

Tell your doctor which diabetes tablets or insulin you took with the meal and at what time, and about steroids, thyroid medicine or a recent illness, all of which push the value up.

Why take the PPBS test?

  • Catches what fasting misses: After-meal sugar rises first in type 2 diabetes.
  • Checks your treatment: Shows whether meal-time tablets or insulin are matched to what you eat.
  • Pairs with fasting: Fasting and PPBS from the same morning give a fuller picture than either alone.
  • Pregnancy: After-meal sugar is what gestational diabetes care targets.

Questions about the PPBS test

What is the post-prandial blood sugar test used for?

It measures blood glucose two hours after a meal to show how well your body handles a food load. It picks up prediabetes and diabetes that a fasting sugar can miss, and it shows whether meal-time diabetes treatment is working.

Do I need to fast for a PPBS test, and how do I prepare?

No fasting is needed. Eat your usual breakfast, note the time of the first bite, and give the sample exactly 2 hours later. Do not eat, snack or smoke in between, and rest instead of walking or exercising, since activity lowers the reading.

What sample is taken and how does home collection work?

A small blood sample from a vein in the arm. A trained phlebotomist comes to your home with a sealed single-use kit at the two-hour mark, draws the sample in a few minutes, and carries it to the lab.

What is the normal range for post-prandial blood sugar?

The report gives one value, post-prandial blood glucose in mg/dL, with a normal range of 70 to 140 mg/dL. Between 140 and 199 mg/dL suggests impaired glucose tolerance, and 200 mg/dL or above is in the diabetic range.

What does a high PPBS result mean?

A two-hour value of 140 to 199 mg/dL means your body is struggling to clear sugar after food, which is prediabetes. A value of 200 mg/dL or more, found twice, meets the definition of diabetes. Your doctor will usually confirm with an HbA1c and a fasting sugar before starting treatment.

Who should get a PPBS test and how often?

Anyone with a borderline fasting sugar or HbA1c, symptoms like thirst and frequent urination, or risk factors such as family history, excess weight or PCOS. People with diabetes usually repeat it every three months alongside a fasting sample, or more often while medication is being adjusted.

How long does the PPBS report take?

The report reaches you on WhatsApp and email in 8 hours from the time the phlebotomist collects the sample at your home. Book a morning slot and you will usually have it the same day.

Should I skip my diabetes tablet before the PPBS test?

No. Take your tablets or insulin with the meal as you normally do, unless your doctor tells you otherwise. The point of the test is to see how your sugar behaves on your usual treatment, and skipping a dose gives a number that does not reflect your daily control.

My fasting sugar is normal but my PPBS is high. What does that mean?

It is a common early pattern in type 2 diabetes. The body can still hold sugar down overnight but cannot cope with a meal. It counts as prediabetes or diabetes depending on the value, and is worth acting on with diet, activity and a repeat test, because the fasting value usually follows within a few years.
