Cortisol treatment
Medical management of high cortisol levels caused by everyday stress
What is cortisol treatment?
Cortisol is the hormone that gets the body ready for demand. It follows a daily rhythm, peaking within an hour of waking and falling through the day to its lowest point at night, and it rises further in response to illness, injury, exertion and stress.
Sustained pressure, broken sleep, irregular meals, night shifts and heavy stimulant use disturb that rhythm. The result people notice is difficulty falling asleep, waking unrefreshed, fatigue through the afternoon, craving, weight settling around the abdomen and a short fuse.
Management begins with establishing whether cortisol is genuinely raised and why, because an elevation coming from a gland, a tumour or steroid medication is a different problem with a different treatment. Where no such cause is found, the work is on sleep, activity, nutrition and the sources of load, which move the rhythm back over weeks to months.
What it involves
There is no procedure and no anaesthetic. This is a clinic consultation of thirty to forty-five minutes with your endocrinologist, and you go home the same day.
They take a full history: your sleep timing and quality, shift pattern, caffeine and alcohol, exercise, weight change, mood, cycles, and every medicine you take, including steroid inhalers, injections, joint injections and skin creams, since these raise cortisol in the body directly and are easily forgotten. They examine your blood pressure, weight, waist, skin and muscle strength.
Measuring cortisol is done on a schedule instead of as a single reading, because the level changes hour by hour. Depending on what is suspected, blood, saliva at a set time at night, or a twenty-four-hour urine collection is arranged, and a suppression test is used to confirm. A plan follows, with a review usually at three months.
Recovery
There is nothing to recover from, and no time off work.
Changes are judged over weeks. A consistent sleep and wake time, morning light, regular meals, resistance and aerobic exercise and reduced evening stimulants typically improve sleep within two to four weeks and daytime energy by six to twelve. Weight around the middle shifts more slowly, over three to six months, and only with sustained change.
Blood pressure, blood sugar and any measurements that were abnormal are rechecked at three months. Where an underlying medical cause was found and treated, recovery is on a different and longer schedule, and your endocrinologist will set that out separately, since the gland on the other side takes months to resume its own output.
Questions about cortisol treatment
How is a high cortisol level actually confirmed?
Not from one blood test. Cortisol changes by the hour, peaks shortly after waking and falls to its lowest at night, and it rises with acute illness, pregnancy, the combined contraceptive pill, alcohol and low mood, so a single raised result is rarely meaningful. Your endocrinologist confirms it with tests that account for the rhythm: a late-night saliva sample, a twenty-four-hour urine collection, or a test that checks whether a small dose of steroid suppresses your morning level as it should. Two abnormal results from different tests are usually needed before anything is treated.
Can stress alone raise my cortisol enough to cause symptoms?
Prolonged stress, broken sleep and shift work do disturb the daily rhythm, and the fatigue, poor sleep, cravings and central weight gain that follow are real. What the evidence does not support is the idea of adrenal burnout as a diagnosis, or of supplements that claim to reset the glands. Your endocrinologist will look for a medical cause first, because missing one matters, and where none is found the treatment is sleep timing, regular meals, exercise, reducing evening stimulants and addressing the load itself. Improvement comes over weeks instead of days.
Could my medicines be causing it?
Frequently, and it is the first thing to check. Steroid tablets are obvious, and steroid inhalers, nasal sprays, joint and back injections, eye drops and strong skin creams also reach the bloodstream and are routinely forgotten when people list their medicines. Some combination creams sold without prescription in India contain a potent steroid and are used for months. Bring every packet, tube and inhaler to your appointment. Never stop a steroid abruptly on your own, since your glands need weeks to resume their own output and a sudden stop can be dangerous.
