Frenuloplasty Surgery
Treatment for a short, tight, or repeatedly injured penile frenulum causing discomfort or tearing
What is frenuloplasty surgery?
The frenulum is the small band of tissue on the underside of the penis that runs from the head down to the shaft. Frenuloplasty lengthens it. Your surgeon divides the tight band across its width and then closes the wound along its length, which converts a short band into a longer one, and the whole operation is confined to that band.
The foreskin itself is kept. This is a small, precise piece of surgery on a few centimetres of tissue, and it leaves a fine scar on the underside that becomes hard to see within a few months.
There is a blood vessel running in the frenulum, so a little bleeding during the operation is routine and your surgeon will secure it. Dissolvable stitches are used, and they need no removal.
What it involves
This is a Day Care procedure performed under a general anaesthetic, so you will be asleep for it. Do not eat for six hours beforehand or drink for two, and arrange for an adult to take you home and stay with you overnight.
The surgery itself takes fifteen to twenty-five minutes. The area is cleaned and draped, the band is divided and closed, and a light dressing goes on.
You will wake in recovery, spend an hour or two on the ward while the anaesthetic wears off and you are able to pass urine, and go home the same day. Expect to be given painkillers and told to keep the dressing on until the following morning. Nobody should drive, sign anything important or make decisions for twenty-four hours after a general anaesthetic.
Recovery
Soreness lasts three to five days and is well controlled with paracetamol. Bruising on the underside is common and looks worse at day three than at day one.
Most men return to desk work after two days and to physical work after a week. Keep the area dry for forty-eight hours, then shower normally and pat dry. Avoid cycling, running and gym work for two weeks.
Sexual activity, including masturbation, waits three to four weeks. This matters more here than in most small operations, because the wound sits exactly where the tissue is stretched, and going back early is the main reason a repair pulls apart or scars down tight again. Erections during the first fortnight will tug at the stitches and are not a problem.
The stitches dissolve over two to four weeks. Your surgeon will see you at around four weeks to confirm the band moves freely and the scar is soft.
Questions about frenuloplasty surgery
How soon can I have sex again?
Three to four weeks, and this is worth taking literally. The wound sits on the part of the penis that is stretched most during sex, so returning early is the main reason a repair pulls apart or heals into a tight scar, and that can undo the operation entirely. The same applies to masturbation. Erections in the first fortnight will tug at the stitches and feel tight, and that is expected. Your surgeon will check the band moves freely at your follow-up at around four weeks and will tell you when to resume.
Will it tear or tighten again?
Most men have a single operation and no further trouble. Two things can go wrong. The wound can heal with a tight scar, leaving the band as short as it was, and a further procedure is then discussed. Or the tissue tears again during sex, which happens mostly to men who resumed before four weeks were up. Where the tightness is part of a foreskin that is tight all round, a lengthening of the band alone is more likely to fall short, and your surgeon will say so at the consultation. Keeping the area moisturised and the scar soft helps.
Will I lose sensation there?
The area is one of the more sensitive parts of the penis and men reasonably worry about this. The operation lengthens the band and does not remove the sensitive tissue, so sensation is preserved in the great majority. Expect the underside to feel numb, tingling or oddly sensitive for the first few weeks while the nerves settle and the swelling goes down; that resolves over one to three months. A small number of men describe a permanently altered feeling along the scar. Most men who had pain or tearing before find sex more comfortable afterwards, and that is the point of the operation.
