What is in it

About this checkup

The Basic yearly panels with vitamin D and vitamin B12 added — eight tests, thirty-two numbers, one fasting draw.

Those two vitamins are worth their own mention. Deficiency in either is common in India and neither shows in a liver, kidney, lipid or thyroid panel. Low vitamin D causes bone and muscle aches, low mood and fatigue, and is near-universal in people who work indoors. Low B12 causes tiredness, tingling in the hands and feet and difficulty concentrating, and is more common in vegetarians and in anyone on long-term metformin or acid-reducing medication.

Both are corrected with supplements, and both are missed by a checkup that stops at the organ panels.

Who it is for

Anyone due a yearly check who has been tired, low or aching without an explanation. Vegetarians and vegans. People on metformin or long-term antacids, both of which lower B12 absorption. Adults who work indoors, cover up outdoors, or are over 50. Anyone whose Basic checkup came back normal while the symptoms stayed.

How often to repeat it

Once a year. If a deficiency is found and treated, your doctor will usually recheck that value at three months to confirm the supplement is working, then return to the yearly cycle.

How to prepare

  • Fast for 10 to 12 hours before the sample. Water is fine.
  • Stop vitamin D or B12 supplements for a week beforehand unless your doctor has said to continue, since a recent dose raises the level and hides a deficiency.
  • Book a morning slot so the fast ends with breakfast.
  • Take your regular medicines as usual, and tell the phlebotomist what you take.
  • Avoid alcohol for 24 hours beforehand.

What it does not look at

No blood count, no HbA1c and no urine test, so anaemia, three-month sugar control and early kidney protein loss are outside this checkup. The Complete checkup adds those three.

Why take it

  • Tiredness usually has a measurable cause: Vitamin D and B12 deficiency together account for a large share of unexplained fatigue, and both are cheap to correct.
  • Numbness and tingling need B12 ruled out: Low B12 damages nerves, and the damage becomes permanent if it is left long enough.
  • Metformin lowers B12 over years: Anyone on it long-term should have the level checked periodically.
  • Bone pain is often vitamin D: Aching in the back, hips and thighs with no injury is a common presentation, and calcium is in this panel to read alongside it.
  • Everything in the Basic checkup, in the same draw: The liver, kidney, lipid, thyroid and fasting sugar panels are all here too.

Questions about IHC Advanced Health Checkup

How is this different from the Basic checkup?

It is the Basic checkup plus 25-hydroxy vitamin D and vitamin B12. The other six panels — liver, kidney, lipid, thyroid, fasting sugar and calcium — are identical.

Should I stop my vitamin supplements before the test?

Stop vitamin D and B12 supplements for about a week beforehand unless your doctor has told you to keep taking them. A dose in the days before the draw raises the reading and can make a real deficiency look like a normal result.

Why is calcium tested along with vitamin D?

Vitamin D controls how much calcium the gut absorbs, so the two are read together. A low vitamin D with a low calcium points somewhere different from a low vitamin D with a normal calcium, and your doctor needs both numbers to tell them apart.

I am vegetarian. Is B12 the test I need?

It is the right starting point. B12 comes almost entirely from animal foods, so long-term vegetarians and vegans are the group most likely to be deficient. If the level is low your doctor may add folate and a blood count to see whether it has begun to affect the red cells.

Do I need to fast for the vitamin tests?

The vitamins themselves do not require it, but the fasting sugar, the lipid profile and the liver panel in this checkup do. One 10 hour fast covers the whole draw.
